QR-87: Customer-Supplied Sample Process

Complete the form according to the instruction and email it TBUSA_QA_Dept@takarabio.com.

Once the completed form is received and approved, a member of our team will email you a prepaid shipping label with additional instructions on how to ship your samples to Takara Bio USA.

QR-243: ESM Sample Collection Sheet

Complete the sample information sheet with the requested information and email it to ESM_Support@takarabio.com.

Please note that this study requires your own PGT-A results from the TE biopsy of each sample. This information can be provided when available.
